چکیده

C A S E A 31 and 1/7–week gestation premature male infant with a birth weight of 1730 g developed sudden onset of desaturations (oxygen saturation 30%), bradycardia (heart rate 40/min) and hypotension (mean blood pressure,15 mm Hg) at 6 days of age. He was born by a cesarean delivery for maternal preeclampsia, received nasal continuous positive airway pressure for 3 days after birth, and was in room air at the time of the acute event. On day 2 of life, a peripherally inserted central venous catheter (PICC) (L-Cath PICC Polyurethane catheter, 28 GA/1.2 F; Argon Medical Devices, Inc. Athens, TX) was inserted through the basilic vein in the right antecubital fossa to provide total parenteral nutrition. Catheter tip was located at the right atrium/superior vena cava (SVC) junction.

Successful pulmonary embolectomy of a saddle pulmonary thromboembolism in a preterm neonate.

Symptomatic pulmonary thromboembolism (PTE) is rare in neonates, and the diagnosis is often made only postmortem. The true incidence is probably underestimated because of its varying presentations, ranging from mild respiratory distress to acute right-heart failure and cardiovascular collapse. We report a sudden cardiorespiratory collapse on day 10 of life in a preterm neonate who was subsequen...

Moderate Hyperbilirubinemia Alters Neonatal Cardiorespiratory Control and Induces Inflammation in the Nucleus Tractus Solitarius

Hyperbilirubinemia (HB) occurs in 90% of preterm newborns. Moderate HB can induce acute neurological disorders while severe HB has been linked to a higher incidence of apneas of prematurity. The present study aimed to test the hypothesis that even moderate HB disrupts cardiorespiratory control in preterm lambs. Two groups of preterm lambs (born 14 days prior to term), namely control (n = 6) and...

Use of targeted neonatal echocardiography to prevent postoperative cardiorespiratory instability after patent ductus arteriosus ligation.

OBJECTIVES To investigate the value of targeted neonatal echocardiography (TnECHO) in predicting cardiorespiratory instability after patent ductus arteriosus (PDA) ligation, and to evaluate the impact of TnECHO-directed care. STUDY DESIGN We reviewed serial echocardiography evaluations of 62 preterm infants after PDA ligation to investigate the relationship between indices of myocardial perfo...

Is the infant car seat challenge useful? A pilot study in a simulated moving vehicle

BACKGROUND AND OBJECTIVE The American Academy of Pediatrics recommends that preterm infants complete a predischarge 'car seat challenge' observation for cardiorespiratory compromise while in a car seat. This static challenge does not consider the more upright position in a car or the vibration of the seat when the car is moving. This pilot study was designed to assess the cardiorespiratory effe...

Cardiorespiratory events detected by home memory monitoring and one-year neurodevelopmental outcome.

OBJECTIVE To determine if infants with cardiorespiratory events detected by home memory monitoring during early infancy have decreased neurodevelopmental performance. STUDY DESIGN Infants (n = 256) enrolled in the Collaborative Home Infant Monitoring Evaluation also completed the Bayley Scales of Infant Development II at 92 weeks' postconceptional age. Infants were classified as having 0, 1 t...
